A quorum (3 or more) present, the following action was taken by
the Committee:
Discussion on what happened when Brad Krause backed the
garbage truck (G-1) into the door frame on the south overhead
door of the big shop on November 27, 1992. Brad didn't make
the door opening on the first try and had to pull ahead to
along side of the door. Brad noticed the damage to the door
frame when he was going to shut the door. Brad explained
that when backing up he misjudged the location of the back of
the truck because of the overhang and limited sight with his
side mirrors.
The committee recommends that the crew in the shop continue
with the practice of having another employee watch while backing
unit out of shop. The operator of such equipment needs to be
aware of the overhang and location of the rear of the truck
when driving ahead or backing up the unit, at all times.
