E F J O S E P P O W C A M P November 12, 2004 Dear Chamber Member: Enclosed you will fred an enlry form for the Distinguished Service Award nominations. The Laurel Chamber of Commerce DSA Committee will choose winners in each of the six categories and will present these winners with awards at the Distinguished Service Award's Banquet to be held on Monday, January 17, 2005. Judging is done on a point basis after all of the nominations are received. The categories are: Employee of the Year, Boss or Manager of the Year, Spanial Service Award, Public Official of the Year, Business Person(s) of the Year, and Lifetime Achievement Award. ls there someone out there that needs to be recognized for his/her efforts? Do you have an employee or do you work with someone who makes it a pleasure to be around? Do you have a boss that is caring, kind or considerate? Is there someone in your organization who does things and does not ever get recognized for their efforts? Is there a police officer, dispatcher, postal worker, teacher, principal, government or city agent, or any other public official who contributes to the wel~re of others above and beyond what is expected of himTher? Is there someone who hms a business that cona'ibutes to the well being of the community just for the sake of making the community a better place in which to live? lfyou know people who fit any of these categories, we would like to hear from you. Feel free to photocopy the nomination form and enter as many people as you wish. You may also want to give ex'Ira copies to your employees for their nominations. Deadline for nominations is December 3 I, 2004. Nomination forms may be dropped offor mailed to the office at: Laurel Chamber of Commerce, 108 East Main, Laurel, MT 59044. Nominations may also be emailed to the office at: seeretarv(~_~lanrelmontana.org or they may be faxed at 628-8105 - call this number before faxing. A banquet will be held on Monday, January 17, 2005 at the Laurel Golf Club. Social hour will be at 6 p.m., dinner at 7 p.m. and awards to follow alter dinner. (Consideration of your nomination may include, but not be limited to the city government, educational system individuals or officials either elected, appointed, or employed where their position includes some aspect of administrative duties.) Business Person of the Year: Contribution to the community and/or business market as a whole, contribution to their employees, or contribution to theircustomers. (Your nomination could include owners or co-owners in the wholesale, manufacturing, retail, food and beverage business as well as legal, professional, or~ serv'ice businesses, including financial institutions, auto dealerships, etc.) Special Sen,ice Award: Contribution to the conmmnity, or their organization or group (if any). (Your nomination could not only include those belonging to an organization or club, but may include anyone who has served the corrm, unity in any form.) Lifetime Achievement Awarrl,: Contribution to the community over a period of many years and has devoted much o~ their life making the community a better place to live or has tried to make a difference in the lives of the people around them.
