INSURANCE COMMITTEE
JULY 15, 1999 10:00 A.M.
OLD COUNCIL CHAMBERS

Dave Allen
The committee met to clarify the dental, vision, and life and ADD plans. Dave
Allen of Western States presented the dental plans through Fortis and the optional
additional vision, which he did not recommend due to cost versus coverage.
Les motioned to recommend the council to accept the Fortis Dental Plan at the
$1,500 maximum with orthodontia, seconded by Bud. Motion passed
unanimously.
Bud motioned to recommend to the council to accept the Fortis Life and ADD at
the $10,000 level for those employees currently qualified, seconded by Mike.
Motion passed unanimously.
Mary motioned that the vision benefits will be the riders offered with the medical
plans, seconded by Les. Motion passed unanimously.
Dirk Kroll, who was not present, was given the information on the 14th and called
to give his consensus with the group.
Meeting adjourned at 10:40 a.m.
